Most initial appointments at Premier Physical Therapy in New York and Connecticut are available within 24-hours.  Please request an appointment online by completing and submitting the form below or call to schedule your preferred appointment at one of our physical therapy centers of excellence located in Manhattan (New York City), Westchester County, NY (Eastchester, Hartsdale & Katonah), and Fairfield County, CT (Darien & Norwalk).
 

Once you have made your initial appointment, please complete our Patient Forms, which will save you time at your first visit.

Current Patients: Please do not use the “Request an Appointment” form for scheduling new appointments; cancelling or making changes to currently scheduled appointments as we cannot accept these through our website. Instead, please call the specific office location where you receive your physical therapy.
